Scale narrows the distribution tube, starves half the evaporator and stretches the cycle until production falls off. A proper descale takes the water system apart: cleaner through it, the tube cleared, pan and sump scrubbed, then sanitizer and full rinse cycles.

A water filter has a capacity measured in gallons. Once it is spent, the ice turns white, melts faster and carries the taste of the water, and the same minerals scale the evaporator.

On an air-cooled machine, a loaded air filter chokes the condenser, the freeze cycle ends early and the bin fills with shells instead of cubes. In a busy kitchen the filter wants looking at monthly.

A full clean and sanitize at least every six months. Busy bars and restaurants should plan on every three to four months, the range manufacturers generally recommend for high-use machines. More in Ice is food.
Usually the water, not the machine. At one bar the water filter was loaded far past its capacity, and a new filter brought back clear cubes. The minerals that cloud the ice also scale the evaporator, so it is worth fixing early.
Hollow or thin cubes mean the freeze cycle is ending before the cube is finished, usually because the machine cannot get rid of heat. Check the air filter first: on one Manitowoc Indigo head, that was the whole problem.
